    New Students
    1. 1. Form 138 (Report Card) duly signed by the Principal
    2. 2. NSO Birth Certificate (photocopy)
    3. 3. Baptismal Certificate (photocopy)
    4. 4. Certification of Good Moral Character
    5. 5. Two copies I.D. pictures (1x1) white background
    6. 6. Junior High School Certificate of Completion
    7. 3. No pending cases in the previous school
    8. 7. NCAE Result
    9. 8. Qualified Voucher Recipient Certificate(for Non-ESC from private schools only)
    10. 9. ESC Certificate (for ESC Grantees from private schools only)

      11. Old Students
      1. 1. Presentation of Form 138 / Report Card
      2. 2. Satisfactory grade in conduct
      3. 3. No academic deficiency
      4. 4. Student’s Contract ( for those who are recommended)
